Catchment area
Before filling out a registration form, check you are in our catchment area (highlighted on the map below). We accept patients who live within a five mile radius of the surgery.

Registering with the surgery
To register with the surgery:
- fill out an online registration form
- print and complete a registration form (PDF, 1.35MB) and bring into reception
- visit reception from and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Travelling community
If you are from the Traveller Community or homeless with no proof of address, you can still register with us if you reside in the local area. If you do not have a proof of ID, but would like to register with us, please come in and have a chat with us.
